Then, you can go over here to fill out and receive your rebate form (apparently the rebate form will be sent to your email within 24 hours after you request it).

So, this offer is good only on purchases made between August 2nd and 8th. Your rebate form must be postmarked no later than 8/31. Limit one per household.
